Registration Checklist: A list of Registration and Troop Dues you are paying for at the first meeting.
Meeting Permission Slip: Fill out this permission slip at the beginning of the year. This is kept on file with the Troop and grants permission for your daughter to attend regular Troop Meetings.
Health & Medical Form: Filled out by both girls and Adult Members/Leaders. Troop keeps this on file for 1 year in case of emergency.
High Adventure Medical Form: The High Adventure Medical Form is required for girls and adults who will be participating in any of AHG's Authorized High Adventure Activities. This form must be signed by a physician.
Troop Transport Form: Transport Form to be filled out by adult drivers and kept on file with Troop records. Must be filled out if transporting girls (other than your own) to any AHG events.
